There is a change to the Civil Rights Compliance process. There is a new Civil Rights Compliance form to be used when submitting a new licensing application, and a new Civil Rights Compliance Renewal form to be used when submitting a licensing renewal application.
Prospective child care providers seeking a new license will utilize the Civil Rights Compliance (CRC) form (HS 2126 9/24). The prior CRC form (HS 2126 8/22) is now obsolete and will be returned to providers if submitted on or after October 1, 2024. The CRC form (HS 2126 9/24) must be included in the packet submitted as part of any new license request.
For licensing renewals, a licensee will now submit the CRC Renewal form (HS 2125 9/24). The prior Attestation form (HS 2125 8/22) is now obsolete and will be returned to providers if submitted on or after October 1, 2024. The CRC Renewal form (HS 2125 9/24) must be submitted as part of a renewal application. The renewal notice will still be sent out 130 days prior to expiration.

*Licensees who use CLS or PELICAN to submit renewal applications must submit the CRC Renewal form by email. The CRC Renewal form is not part of the electronic CLS/PELICAN renewal application process.
The new process is effective on Tuesday, October 1, 2024. Existing licensees and applicants must begin following the new process on or before that date.
